Bereavement Coordinator

Medical Director

Our hospice Medical Director, along with your child's pediatrician if he or she has one, ensures that your child's care plan meets his or her unique needs. Our physicians are also available to make house calls should your child need one. If your child does not have a pediatrician, you may choose to have a Heart of Hospice physician manage his or her care.

Bereavement Coordinator

Our bereavement coordinators provide support and guidance to your family and loved ones.

Registered Nurse

Your Heart of Hospice nurse works with your pediatrician to manage your child's medications and other treatments to keep him or her comfortable. Your child's nurse is available to answer any questions you may have.

Social Worker

Our social workers help connect you and your child with community resources such as financial assistance. Heart of Hospice social workers can also provide emotional support for you and your child.

Chaplain

Our chaplain can provide spiritual support for you and your child, regardless of your faith tradition or beliefs. Heart of Hospice chaplains can also coordinate care with your current clergy if you prefer.

Nurse Practitioner

Our nurse practitioners are available to make house calls and manage any pain or symptoms of discomfort by coordinating with our hospice Medical Director and your child's pediatrician when applicable.

Hospice Aide

Our certified hospice aides help your child with activities of daily living such as bathing, grooming, and dressing. Heart of Hospice aides offer extra attention if you desire, such as nail care, reading, or companionship.

Volunteers

Heart of Hospice volunteers are trained to provide companionship for your child and support for you so you and your family can rest, run errands, or do any other activities you might find helpful.
